Eligibility

Employees are eligible to enroll on the first of the month following 60 days of employment. Previously Eligible Employees can also enroll at open enrollment, which is the end of October. A life change or qualifying event does allow you to change or add coverage.

Types of Coverage

• Supplemental Insurance - Guardian, includes;

• Voluntary Long Term Disability

• Voluntary Term Life Insurance

• Short Term Disability

Fitz & Smith Inc. covers 100% of the cost for short-term disability and vision for each employee that adds this benefit at enrollment.
